Warfarin
NSAIDs increase anticoagulant effect of warfarin and risk of GI bleeding. Monitor INR closely.
Methotrexate
NSAIDs reduce methotrexate clearance, potentially causing serious toxicity. Avoid combination.
Aspirin
Naproxen does not block aspirin's irreversible antiplatelet effect but may compete for COX binding. Take aspirin ≥30 min before naproxen. Avoid long-term combination.
ACE inhibitors / ARBs (lisinopril, losartan)
Reduced antihypertensive efficacy; increased risk of acute kidney injury with volume depletion (triple whammy effect with diuretics).
Diuretics (furosemide, hydrochlorothiazide)
Reduced diuretic efficacy; increased risk of nephrotoxicity.
Lithium
NSAIDs reduce lithium renal clearance, increasing lithium plasma levels up to 20%. Monitor lithium levels.
SSRIs (sertraline, fluoxetine, escitalopram)
Increased risk of GI bleeding — SSRIs reduce platelet serotonin, impairing hemostasis.
Corticosteroids (prednisone)
Additive GI toxicity. Use GI prophylaxis (PPI) if combination required.
