OTC (Aleve): 220 mg every 8–12 hours. Maximum 660 mg/day. Prescription — Arthritis/pain: 250–500 mg (naproxen) twice daily or 375–500 mg twice daily. Gout: 750 mg initially, then 250 mg every 8 hours until attack subsides. Maximum prescription dose: 1500 mg/day (under physician supervision). Extended-release (Naprelan): 750–1000 mg once daily.
Children ≥2 years (prescription only): 5 mg/kg twice daily for juvenile idiopathic arthritis. OTC not recommended under 12 years.
Avoid use with eGFR <30 mL/min. Use caution with moderate renal impairment. Use the lowest effective dose and monitor renal function.
Caution in hepatic impairment. Avoid in severe hepatic disease.
