Your healthcare provider will determine the appropriate dose based on your specific condition, kidney function, other medications, and medical history. Hypertension: 10 mg once daily. Usual range: 20–40 mg/day. Maximum: 80 mg/day. Heart failure: Start 2.5–5 mg once daily, titrate to 40 mg/day. Post-MI: 5 mg within 24 hours, then 5 mg after 24 hours, 10 mg after 48 hours, then 10 mg daily.
Children ≥6 years (hypertension): 0.07 mg/kg once daily (up to 5 mg). Max: 0.6 mg/kg/day (up to 40 mg/day).
CrCl 10–30 mL/min: Start 2.5–5 mg/day. CrCl <10 mL/min: Start 2.5 mg/day. Dialysis patients: Initial 2.5 mg after each dialysis session.
