Methotrexate

Probenecid
Markedly increases ketorolac plasma levels. Concomitant use is contraindicated.
ACE inhibitors / ARBs
Reduced antihypertensive effect; increased risk of acute kidney injury, especially with volume depletion.
Diuretics (furosemide, hydrochlorothiazide)
Reduced diuretic effect; increased nephrotoxicity risk.
Lithium
NSAIDs reduce lithium renal clearance, increasing lithium levels and toxicity risk. Monitor levels.
Pentoxifylline
Increased risk of bleeding.
SSRIs
Increased GI bleeding risk due to reduced platelet serotonin.
