IV: 15–30 mg every 6 hours. IM: 30–60 mg single dose or 15–30 mg every 6 hours. Oral: 10 mg every 4–6 hours (maximum 40 mg/day). Maximum combined duration (all routes): 5 days. Elderly (≥65 years): Reduce doses by 50%. Nasal spray: 1 spray (15.75 mg) each nostril every 6–8 hours; max 4 doses/day.
Safety and efficacy not established in children <17 years for systemic use.
Reduce dose in renal impairment. Contraindicated in advanced renal disease or risk of renal failure. Avoid with eGFR <30 mL/min.
Use with caution in hepatic impairment. Avoid in severe hepatic disease.
