Hypoglycemia: All sulfonylureas can cause hypoglycemia — the most significant risk. Unlike metformin, sulfonylureas cause insulin release even when blood glucose is normal or low. Risk factors: skipped meals, excessive exercise, renal/hepatic impairment, alcohol use, medicine interactions, elderly patients. Counsel patients to always carry glucose tablets or juice. Severe hypoglycemia may require IV dextrose.
Cardiovascular Risk: The UKPDS trial and subsequent data raised questions about cardiovascular safety of sulfonylureas (particularly the ACCORD trial outcome with glyburide). Newer agents (GLP-1 RAs, SGLT2 inhibitors) have shown positive cardiovascular outcomes. Sulfonylureas lack cardiovascular outcome trial benefit evidence. Consider alternative agents in high cardiovascular risk patients.
Elderly Patients: Sulfonylureas are particularly risky in elderly patients due to reduced renal clearance, irregular meal schedules, and impaired hypoglycemia recognition. The Beers Criteria lists glyburide (not glipizide) as potentially inappropriate; glipizide is preferred among sulfonylureas in elderly due to shorter duration. Start at 2.5mg.
