Thioridazine
Duloxetine inhibits CYP2D6; thioridazine is a CYP2D6 substrate; combination increases QTc risk.
Management: Contraindicated.
Warfarin / anticoagulants
Increased bleeding risk via antiplatelet effects. SNRIs inhibit platelet serotonin.
Management: Monitor INR closely; instruct patients to report unusual bleeding.
NSAIDs / aspirin
Additive bleeding risk.
Management: Use with caution; consider PPI gastroprotection.
