Tooth Discoloration and Enamel Hypoplasia in Children: Doxycycline (like all tetracyclines) binds to calcium in developing teeth and bone, causing permanent yellow-brown discoloration and enamel hypoplasia. CONTRAINDICATED in children under 8 years and in second/third trimester pregnancy. Exception: Rocky Mountain spotted fever — the benefit of treating RMSF far outweighs the risk of tooth discoloration at any age, and RMSF is fatal if untreated.
Photosensitivity: Doxycycline is one of the strongest photosensitizing antibiotics. Patients must use broad-spectrum sunscreen (SPF 30+), protective clothing, and avoid prolonged sun exposure while on doxycycline. Severe phototoxic reactions (exaggerated sunburn, blisters) can occur with minimal sun exposure.
Take with Food and Water: To prevent esophageal ulceration, take doxycycline with a full 8oz glass of water and remain upright for 30–60 minutes after dosing. Do not take immediately before lying down or at bedtime. Taking with food reduces GI upset but may slightly reduce absorption.
Hepatotoxicity Risk with IV: High-dose IV doxycycline (used in ICU) carries risk of hepatotoxicity. Monitor liver function tests.
