Potentially life-threatening or causing permanent damage. Avoid combination.
May worsen condition or require dose adjustment. Monitor closely.
Usually limited clinical effect. Manage with routine monitoring.
MAOIs
Risk of hypertensive reaction and seizures. 14-day washout required.
Management: Contraindicated with MAOIs or within 14 days of stopping MAOI.
Alcohol / benzodiazepine withdrawal
Bupropion lowers seizure threshold. Risk of seizures increased with alcohol or benzodiazepine withdrawal.
Management: Avoid in patients with alcohol abuse history or benzodiazepine dependence (due to withdrawal seizure risk).
CYP2D6 substrates (codeine, tamoxifen, TCAs, antipsychotics, beta-blockers)
Bupropion is a potent CYP2D6 inhibitor. Can increase levels of medicines metabolized by CYP2D6.
Management: Monitor for increased effects/toxicity of CYP2D6 substrates; dose adjustments may be needed.
Linezolid / serotonergic medicines
Some serotonergic potential via dopamine pathways.
Management: Monitor carefully if combination unavoidable.
Always ask your pharmacist about potential interactions with food, alcohol, and supplements specific to Bupropion. Some medicines have significant interactions with grapefruit juice, high-fat meals, dairy products, or vitamin K-rich foods.
