MDD (IR): 100mg twice daily initially; increase to 100mg TID after 3 days; max 450mg/day in 3 divided doses (max single dose 150mg); MDD (SR): 150mg once daily morning x 3 days; then 150mg twice daily; max 400mg/day; MDD (XL): 150mg once daily morning; may increase to 300mg after 4 weeks; max 450mg/day; SAD (XL): 150mg once daily autumn; may increase to 300mg after 1 week; taper in spring; Smoking cessation (Zyban SR): 150mg once daily x 3 days; then 150mg twice daily for 7–12 weeks
Not FDA-approved for depression in children. Off-label ADHD use: 3–6 mg/kg/day.
Use with caution in moderate/severe renal impairment — active metabolite can accumulate.
Reduce dose in hepatic impairment. Severe hepatic impairment: max 75mg/day (IR) or 100mg every other day (SR/XL).
