AFib (stroke prevention): 20 mg once daily with evening meal. DVT/PE treatment: 15 mg twice daily with food × 21 days, then 20 mg once daily with food. DVT/PE prevention: 10 mg once daily. Post-hip replacement: 10 mg once daily × 35 days. Post-knee replacement: 10 mg once daily × 12 days. CAD/PAD (with aspirin 100 mg): 2.5 mg twice daily.
Pediatric VTE treatment: Weight-based dosing (0.8–1.6 mg/kg/day based on weight and indication). Available as granules for oral suspension.
AFib: Avoid if CrCl <15 mL/min. DVT/PE treatment: Avoid if CrCl <30 mL/min. VTE prophylaxis: Avoid if CrCl <30 mL/min. Dose adjustment tables vary by indication.
Contraindicated with hepatic disease (Child-Pugh B or C) associated with coagulopathy and bleeding risk.
