Nausea (24–40%) — most common, especially on initiation
Dizziness (10–33%)
Constipation (10–46%)
Headache (18–32%)
Drowsiness/sedation (7–25%)
Vomiting (5–17%)
Dry mouth (5–13%)
Seizures — risk increased with doses above recommended; serotonergic potential lowers seizure threshold
Serotonin syndrome (with SSRIs, SNRIs, MAOIs, triptans, other serotonergic medicines)
Respiratory depression (especially in opioid-naive patients or when combined with CNS depressants)
Medicine dependence and withdrawal syndrome
Hypoglycemia (rare but documented)
Adrenal insufficiency (rare, class effect with opioids)
Androgen deficiency with prolonged use
QTc prolongation (rare at standard doses)
