Immediate release: 50–100mg every 4–6 hours; max 400mg/day (300mg/day in patients >75 years); Extended release (ConZip): 100mg once daily; titrate by 100mg every 5 days; max 300mg/day; Combination (Ultracet): Tramadol 37.5mg/acetaminophen 325mg: 2 tablets every 4–6 hours; max 8 tablets/day
NOT approved for pediatric use. FDA warning against use in children <12 years; contraindicated post-tonsillectomy in children <17 years.
CrCl 10–30 mL/min: increase interval to every 12 hours. Dialysis: every 12 hours; max 200mg/day.
Severe impairment: 50mg every 12 hours.
