Intraoperative Floppy Iris Syndrome (IFIS): Alpha-1 blockers, including tamsulosin, have been associated with IFIS during cataract surgery — the iris may billow and prolapse during surgery, causing complications. IFIS can occur even years after stopping tamsulosin. Patients MUST inform their ophthalmologist/surgeon before cataract surgery that they have ever taken tamsulosin, even if stopped previously. Surgeons should plan modifications to surgical technique.
Postural Hypotension: Although tamsulosin is more uroselective than other alpha-blockers, orthostatic hypotension can still occur, particularly with the first dose, after dose increase, or in dehydrated patients. Counsel patients to rise slowly.
Not for Female Use: Tamsulosin is not approved for use in women. However, it is increasingly used off-label in women for bladder outlet obstruction and kidney stone expulsion.
Effect on Ejaculation: Retrograde ejaculation (semen goes backward into bladder rather than forward) occurs in 8–18% of patients. This is not harmful but may be unexpected. Patients planning to father children should be informed.
