Heart failure (HFrEF): 12.5–25mg once daily initially; target 25–50mg once daily; Resistant hypertension: 25–50mg once daily; max 100mg/day; Cirrhotic ascites: 100mg/day initially; may increase to 400mg/day; typically combined with furosemide; Primary hyperaldosteronism: 100–400mg/day (diagnosis/preoperative); long-term 100mg/day if not surgical; Acne/hirsutism (off-label): 50–200mg/day
Edema/heart failure: 1–3.3mg/kg/day in 1–2 divided doses.
Contraindicated in significant renal impairment (CrCl <30 mL/min) or hyperkalemia. Risk of severe hyperkalemia.
Use with caution. Often the preferred diuretic in cirrhosis (ascites) but monitor electrolytes.
