Schizophrenia (acute): 2mg/day on day 1; 4mg/day on day 2; titrate to 4–8mg/day; max 16mg/day (though doses above 8mg rarely provide additional benefit and increase side effects); Schizophrenia (maintenance): 4–8mg once daily; Bipolar I (acute mania): 2–3mg once daily initially; titrate by 1mg/day; range 1–6mg/day; Risperdal Consta (IM depot): 25mg IM every 2 weeks; may increase to 37.5mg or 50mg every 2 weeks after 4 weeks; Perseris (subcutaneous depot): 90–120mg monthly
Schizophrenia (13–17 years): 0.5mg/day; target 3mg/day; max 6mg/day. Bipolar (10–17 years): 0.5mg/day; target 1–2.5mg/day; max 6mg/day. Autism irritability (5–16 years): <20kg: 0.25mg/day; ≥20kg: 0.5mg/day; max 3mg/day.
Start at 0.5mg twice daily; titrate slowly. Increase with caution. Use with caution in CrCl <30 mL/min.
