Cardiac Effects: Levothyroxine should not be used for weight loss — supraphysiologic doses can cause serious cardiovascular adverse effects including arrhythmias, angina, and myocardial infarction, especially in elderly patients with underlying heart disease. Adrenal Insufficiency: In patients with adrenal insufficiency, initiating levothyroxine can precipitate adrenal crisis. Treat adrenal insufficiency before or simultaneously with levothyroxine.
