Postherpetic neuralgia: 300mg on Day 1, 600mg on Day 2, 900mg on Day 3; titrate to 1800–3600mg/day in 3 divided doses; Seizures (adjunctive): Start 300mg TID; titrate to 900–1800mg/day in 3 divided doses (max 3600mg/day); Neuropathic pain (off-label): 100–300mg TID; titrate to 900–3600mg/day
Seizures (3–11 years): 10–15 mg/kg/day in 3 divided doses.
Dose reduction required based on CrCl: CrCl 30–59: 400–1400mg/day; CrCl 15–29: 200–700mg/day; CrCl <15: 100–300mg/day.
No adjustment required (not metabolized by liver).
