Hypokalemia (potassium wasting — most important electrolyte effect)
Hyponatremia
Hypomagnesemia
Hypocalcemia (at high doses)
Dehydration and volume depletion
Orthostatic hypotension
Increased thirst
Severe electrolyte imbalances (can cause cardiac arrhythmias)
Acute kidney injury (prerenal azotemia from over-diuresis)
Ototoxicity (hearing loss and tinnitus — especially with high IV doses or concurrent aminoglycosides)
Hyperuricemia and gout attacks (uric acid retention)
Severe skin reactions (Stevens-Johnson syndrome — rare)
Blood dyscrasias (rare)
Pancreatitis (rare)
Hepatic encephalopathy in cirrhotic patients
