Oral (edema): 20–80mg once or twice daily; may increase by 20–40mg every 6–8 hours to maximum 600mg/day in severe cases; IV (acute heart failure/pulmonary edema): 40–80mg IV bolus; repeat/double dose if no response in 1 hour; continuous infusion: 5–40mg/hour; Hypertension: 20–40mg twice daily; Hypercalcemia: 80–100mg IV every 1–2 hours with aggressive IV normal saline hydration
1–2mg/kg/dose orally every 6–12 hours; 0.5–1mg/kg IV every 6–8 hours; maximum 6mg/kg/day.
Higher doses often required in renal impairment to overcome protein binding and achieve tubular secretion. In severe renal impairment, doses up to 160–200mg IV may be needed.
Use with caution in cirrhosis. Start low (20mg) and titrate cautiously; hepatic encephalopathy can be precipitated by electrolyte disturbances.
