AAdult Dosing

Initial: 24/26 mg twice daily; Target: 97/103 mg twice daily; can increase every 2-4 weeks based on tolerance

PPediatric Dosing

Age 1 month to <18 years: dose based on weight, typically 0.6 mg/kg twice daily

RRenal Dose Adjustment

eGFR <30 mL/min or dialysis: 24/26 mg twice daily; increase cautiously

HHepatic Dose Adjustment

Moderate hepatic impairment: 24/26 mg twice daily

General Dosing Tips

Missed dose: Take the missed dose as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular schedule. Do not double up doses.
