CYP2C19 Poor Metabolizers (Boxed Warning): Clopidogrel is a prodrug requiring CYP2C19 activation to its active metabolite. Poor metabolizers (2–14% of patients, depending on ethnicity) have reduced active metabolite formation and reduced antiplatelet effect — potentially increasing cardiovascular events. Tests are available to identify poor metabolizers. Alternative antiplatelet therapy (prasugrel, ticagrelor) should be considered for CYP2C19 poor metabolizers in high-risk situations.
Bleeding Risk: Clopidogrel increases bleeding risk. Any surgery requiring discontinuation should be planned in advance. Clopidogrel should be stopped 5–7 days before elective surgical procedures (CABG: ideally 5 days before) to allow platelet recovery.
TTP: Thrombotic thrombocytopenic purpura (TTP) has been reported within 2 weeks of starting clopidogrel. Presents with thrombocytopenia, microangiopathic hemolytic anemia, neurological symptoms, renal dysfunction, fever. Requires urgent plasmapheresis — discontinue clopidogrel immediately.
Hypersensitivity Cross-Reactivity: Some patients allergic to ticlopidine may react to clopidogrel (thienopyridine class).
