Uncomplicated UTI: 250mg twice daily for 3 days; XR 500mg once daily for 3 days; Complicated UTI/pyelonephritis: 500mg twice daily for 7–14 days; XR 1000mg daily for 7–14 days; Pneumonia: 500–750mg twice daily for 7–14 days; Bone/joint infections: 500–750mg twice daily for 4–6 weeks; Anthrax (post-exposure): 500mg twice daily for 60 days; Traveler's diarrhea: 500mg twice daily for 1–3 days
Limited use due to arthropathy risk. Approved for complicated UTIs and pyelonephritis in children 1–17 years: 10–20mg/kg every 12 hours (max 750mg/dose). Anthrax: 15mg/kg every 12 hours.
CrCl 30–50 mL/min: 250–500mg every 12 hours. CrCl <30 mL/min: 250–500mg every 18 hours. Hemodialysis: 250–500mg every 24 hours after dialysis.
No dose adjustment in mild-moderate impairment.
