Do Not Abruptly Discontinue: Abrupt discontinuation of carvedilol can cause rebound hypertension, angina, and potentially precipitate myocardial infarction. Taper over 1–2 weeks when discontinuing in patients with coronary artery disease.
Initiate in Stable Heart Failure Only: Carvedilol should be initiated ONLY in stable heart failure patients who are not fluid-overloaded. Starting during acute decompensation causes worsening heart failure. Start at the lowest dose (3.125mg twice daily) and titrate slowly over weeks.
Bronchospasm: Non-selective beta blockade can cause bronchospasm in patients with asthma or COPD. Avoid in patients with bronchospastic conditions. If required for compelling cardiac indication, use extreme caution.
Masking of Hypoglycemia: In diabetic patients, carvedilol blunts the tachycardia response to hypoglycemia, masking this symptom. Sweating may still occur. Monitor blood glucose carefully.
Peripheral Vascular Disease: Beta-blockers including carvedilol can worsen peripheral arterial disease symptoms. Use with caution.
