AAdult Dosing

General hyperlipidemia: 20–40mg once daily in the evening; max 40mg/day for most patients; High cardiovascular risk: 40mg once daily in the evening; Familial hypercholesterolemia: 40mg once daily

PPediatric Dosing

Heterozygous FH (10–17 years): 10–40mg once daily in the evening.

RRenal Dose Adjustment

Significant renal impairment: start at 5mg/day and monitor carefully.

HHepatic Dose Adjustment

CONTRAINDICATED in active liver disease or unexplained persistent elevated transaminases.

General Dosing Tips

Missed dose: Take the missed dose as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular schedule. Do not double up doses.
