Insulin
Combination substantially raises risk of severe hypoglycemia.
Management: Reduce insulin when starting oral semaglutide; frequent glucose monitoring required.
Proton pump inhibitors / H2 blockers
Elevated gastric pH reduces oral semaglutide absorption, as SNAC requires an acidic environment.
Management: Avoid PPIs and H2 blockers within 2 hours of Rybelsus dosing.
Sulfonylureas
Additive glucose lowering increases hypoglycemia risk.
Management: Reduce sulfonylurea dose; monitor blood glucose closely.
Other oral medications (taken within 30 min of dose)
Rybelsus must be taken on an empty stomach with plain water; any food, drink, or other medication within 30 minutes significantly reduces semaglutide absorption.
Management: Wait at least 30 minutes after taking Rybelsus before eating, drinking, or taking other medications.
Warfarin
Altered gastric emptying and absorption changes may affect INR.
Management: Monitor INR when starting or adjusting oral semaglutide dose.
Always ask your pharmacist about potential interactions with food, alcohol, and supplements specific to Rybelsus. Some medicines have significant interactions with grapefruit juice, high-fat meals, dairy products, or vitamin K-rich foods.
