Gemfibrozil
Inhibits rosuvastatin transport (OATP1B1), increasing rosuvastatin plasma levels 2-fold and myopathy risk.
Management: Avoid rosuvastatin + gemfibrozil. If fibrate needed, use fenofibrate.
Cyclosporine
Cyclosporine inhibits OATP1B1 transport, increasing rosuvastatin AUC 7-fold.
Management: Maximum 5mg/day in patients on cyclosporine.
Warfarin
Rosuvastatin can increase anticoagulant effect of warfarin.
Management: Monitor INR when initiating rosuvastatin or changing doses.
Antacids (aluminum/magnesium hydroxide)
Reduces rosuvastatin Cmax by 54% and AUC by 37% when taken simultaneously.
