Schizophrenia (IR): 25mg twice daily; titrate to 300–400mg/day in 2–3 divided doses; range 150–750mg/day; Bipolar mania (IR): 50mg twice daily on day 1; 100mg twice daily day 2; titrate to 400–800mg/day; Bipolar depression (XR): 50mg once daily at bedtime; titrate to 300mg/day over 4 days; Adjunctive MDD (XR): 50mg once daily at bedtime; may increase to 150–300mg/day; Off-label insomnia: 25–50mg at bedtime
Schizophrenia (13–17 years): 25mg twice daily; target 400–800mg/day. Bipolar mania (10–17 years): 25mg twice daily; target 400–600mg/day.
No dose adjustment required.
Start at 25mg/day and increase slowly in hepatic impairment.
