GERD (erosive esophagitis): 40mg once daily for 8 weeks; maintenance 40mg once daily; Pathological hypersecretory conditions: 40mg twice daily initially; may increase to 240mg/day; H. pylori (triple therapy): 40mg twice daily with amoxicillin 1g and clarithromycin 500mg for 10–14 days; IV (stress ulcer prophylaxis): 40mg IV daily or 40mg IV every 12 hours for high-risk ICU patients
≥5 years (GERD): 20mg or 40mg once daily based on weight (≥15kg: 20mg; ≥40kg: 40mg).
No dose adjustment required.
No dose adjustment in mild-moderate hepatic impairment. Maximum 40mg/day in severe impairment.
