GERD (symptomatic relief): 20mg once daily for 4 weeks; Erosive esophagitis (healing): 20mg once daily for 4–8 weeks; H. pylori (triple therapy): 20mg twice daily for 10–14 days (with antibiotics); Duodenal ulcer: 20mg once daily for 4 weeks; Gastric ulcer: 40mg once daily for 4–8 weeks; Zollinger-Ellison syndrome: 60mg once daily; titrate as needed (up to 120mg TID); NSAID ulcer prevention: 20mg once daily
GERD (1–16 years): 5–20mg once daily based on weight.
No dose adjustment required.
Use lowest effective dose in severe hepatic impairment; maximum 20mg/day recommended.
