C. difficile (non-severe): 500mg three times daily for 10–14 days (oral; vancomycin now preferred per guidelines for most patients); Bacterial vaginosis: 500mg twice daily for 7 days; or 2g single dose (lower cure rate); Trichomonas: 2g single dose; or 500mg twice daily for 7 days. Treat sexual partners simultaneously.; Giardia: 250mg three times daily for 5–7 days; or 2g once daily for 3 days; H. pylori (triple therapy): 500mg twice daily with PPI and amoxicillin/clarithromycin for 10–14 days; Anaerobic infections: 500mg every 6–8 hours IV/orally
Bacterial infections: 7.5mg/kg every 8 hours (max 500mg/dose). Trichomoniasis: 15mg/kg/day in 3 doses for 7 days.
No dose adjustment for mild-moderate renal impairment. In severe impairment, extend interval.
Reduce dose in severe hepatic impairment — hepatic metabolism is primary elimination pathway.
