Abrupt Discontinuation: Do NOT abruptly stop metoprolol, especially in patients with CAD. Abrupt withdrawal can cause rebound angina, MI, and hypertension. Taper over 1–2 weeks.
Heart Failure: The succinate (XL) formulation is the ONLY formulation with proven mortality benefit in heart failure. Initiate at low doses and titrate slowly in stable patients.
Bronchospasm: Even cardioselective beta-blockers can trigger bronchospasm. Use with extreme caution (or avoid) in patients with asthma or reactive airway disease.
Peripheral Vascular Disease: Can worsen claudication symptoms.
Diabetes: Masks signs of hypoglycemia (particularly tachycardia); sweating may be preserved.
