MAOIs
Dangerous hypertensive crisis risk. Contraindicated.
Management: Contraindicated during or within 14 days of MAOI therapy.
Antihypertensives
Methylphenidate increases blood pressure and heart rate, potentially counteracting antihypertensive therapy.
Management: Monitor blood pressure; antihypertensive dose adjustment may be needed.
Anticoagulants (warfarin) / certain antidepressants (TCAs)
Methylphenidate inhibits metabolism of warfarin, TCAs, and some anticonvulsants via CYP2D6 and other pathways.
Management: Monitor medicine levels and effects when starting or stopping methylphenidate.
Alcohol
Alcohol can increase peak methylphenidate concentrations. Also impairs sleep, counteracting ADHD treatment benefits.
Management: Counsel patients to minimize or avoid alcohol.
Always ask your pharmacist about potential interactions with food, alcohol, and supplements specific to Methylphenidate. Some medicines have significant interactions with grapefruit juice, high-fat meals, dairy products, or vitamin K-rich foods.
