GI upset (nausea, dyspepsia, heartburn) — 10–20%
Abdominal pain
Dizziness
Headache
Edema (fluid retention)
Elevated blood pressure
GI ulcers and bleeding (3–4x increased risk vs. placebo; risk increases with dose and duration)
Cardiovascular events: myocardial infarction, stroke (boxed warning — risk increases with duration of use)
Acute kidney injury (especially in volume-depleted, elderly, or concurrent RAAS inhibitor use)
Serious skin reactions: Stevens-Johnson syndrome, toxic epidermal necrolysis (rare)
Hepatotoxicity (rare; elevations in LFTs more common)
Aseptic meningitis (rare, especially in lupus patients)
Bronchospasm / aspirin-exacerbated respiratory disease (AERD)
Aplastic anemia, agranulocytosis (very rare)
Cognitive effects in elderly
