Strong CYP3A4 inhibitors (clarithromycin, itraconazole, HIV protease inhibitors)
Significantly increase atorvastatin levels, greatly increasing myopathy/rhabdomyolysis risk. Avoid or limit atorvastatin to 20 mg.
Cyclosporine
Markedly increases atorvastatin exposure. Atorvastatin doses should be limited to 10 mg/day.
Gemfibrozil / fibrates
Combined fibrate and statin use increases myopathy and rhabdomyolysis risk. Avoid if possible.
Colchicine
Increased risk of myopathy when combined with statins.
Amiodarone
May increase atorvastatin levels and myopathy risk.
Warfarin
Atorvastatin may modestly increase INR. Monitor anticoagulation.
Digoxin
Atorvastatin slightly increases digoxin levels. Monitor digoxin.
Oral contraceptives
Atorvastatin may increase estrogen and norethindrone levels.
