Mild to moderate infections: 500mg every 8 hours OR 875mg every 12 hours; Severe infections: 500mg every 6 hours OR 875mg every 8 hours; H. pylori (triple therapy): 1000mg twice daily for 10–14 days (with clarithromycin + PPI); Dental prophylaxis: 2g single dose 30–60 minutes before procedure; Strep pharyngitis: 1000mg once daily OR 500mg twice daily for 10 days
Otitis media: 80–90 mg/kg/day in 2 divided doses (high-dose). Standard: 25–45 mg/kg/day in 2–3 divided doses.
CrCl 10–30: 250–500mg every 12h. CrCl <10: 250–500mg every 24h.
Use with caution in severe hepatic impairment.
